Our Office Policies

We have your convenience in mind. With the NEW online patient registration portal you can fill out your patient registration any time of day. Registering online will save time. Registration only takes about 10 minutes. Before you start the registration process, have your medical insurance cards at hand. Fill out the information as accurately and completely as possible. Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice.
Please extend us this courtesy so that other patients may be seen.

Emergencies

Our office provides on-call service for emergencies. Call 570-843-1972 so that the physician on call can be contacted.

In the even of a medical emergency, call 911.

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. 


All co-payments and deductibles are due at the time of service. For your convenience, we accept all major credit cards as well as PayPal.
We will make all reasonable attempts to bill your insurance company, However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
